The Tripp Community Care Collaborative is a nonprofit community organization serving elders and their caregivers in the Pioneer Valley (Franklin and Hampshire Counties) of Western Massachusetts.
Our Mission is to improve the ability of our community to care for our elders and therefore for one another in a compassionate, cost-effective, collaborative, and sustainable manner through advocacy, caregiver education and training, care coordination, community outreach, and strengthening existing networks of care.
OUR PROGRAMS
Caregiver Education and Training Our goal is to enhance the ability of caregivers and the community to meet the needs of our elders: ♦ by providing certification courses for nursing assistants and home health care aides
Caregiver Assessment & Coordination Our goal is to help elders receive the coordinated care they need to maintain optimum quality of life in their own homes: ♦ by providing care assessments, consultation, and coordination conducted by experienced geriatric professionals and ♦ by coordinating elders' care-related services and ensuring effective teamwork among caregivers.
Community Outreach and Education Our goal is to increase community awareness of the needs of ouir elders and their caregivers: ♦ by enabling elders to contribute their wisdom and knowledge to our communities, ♦ by educating the community about regulatory and governmental policies that directly affect our elders, their families, their caregivers, and ultimately, all of us, and ♦ by filling the gaps in collaborative care services for elders with innovative programs such as storytelling and creative writing workshops for caregivers and elders, providing spiritual care to elders at home, community forums, and media outreach.
